We appreciate referrals in any shape or form, however, you might find it handy to add our on-line referral form to the favourites list on your surgery p.c.; your staff can fill it out while the patient is in the chair. The referral form prompts you to request copies of digital photos and radiographs for your patients - if you still need such copies for other patients just let us know.
